1.Your lawful permanent residency card(Green Card) 2.State Identification or Driver’s License 3.All police and court records if you have been arrested. cited, detained, or charged 4.If applying for waivor, please bring a verification of benefits letter and/or a copy of your most recent tax return

INFORMATION:

1.Addresses, work, and schooling (last 5 years) 2. All trips outside of the U.S. (last 5 years);  and current and expired passports 3.Address and date of birth of all your children
